Description of events
At about 2:45 p.m., M. Alvarez was walking down the stairs from the shipping office carrying a box of labels. The metal nosing on the third step from the bottom was loose; it shifted under the right foot and the knee twisted as M. Alvarez caught the handrail. R. Singh, coming up the stairs, saw the fall and helped M. Alvarez sit down. L. Chen applied an ice pack and drove M. Alvarez to urgent care at 3:20 p.m. The step was taped off at 3:05 p.m. and maintenance was called. Photos of the loose nosing were taken before it was repaired.

How to file a defensible company / hr report
- 1
Record the circumstances promptly
Document what happened, where, and what the employee was doing at the time, as soon as it's reported to you.
- 2
Interview witnesses
Note anyone who saw the accident: their account is part of the record, not just the employee's own.
- 3
Identify the root cause
Use the dedicated root-cause field to note what actually caused it, not just what happened, so the fix addresses the real problem.
- 4
Log the corrective action taken
Record what was changed or fixed afterward. This is what turns a one-off report into evidence of a functioning safety program.
Common use cases
- On-duty accident
- Medical incident or illness at the office
- Accident involving office equipment
- Incident during a business trip
Frequently asked questions
How should I structure corrective actions?
The form has dedicated "Immediate action" and "Root cause" fields, so you document not just the incident but also the steps taken to prevent it from happening again.
What is an employee accident report?
It's the written record of an accident that injured an employee or could have: what happened, when and where, who was involved, the injury and the first aid or medical care, witnesses, and the steps taken to prevent it from happening again. It supports the workers' comp claim and, when the case is recordable, the OSHA 300 log.
Who should fill out an employee accident report?
The injured employee, if they're able to, with their supervisor. The employee describes what happened in their own words; the supervisor adds the immediate actions and the cause; witnesses give their own accounts and sign. If the employee can't write it, the supervisor fills it in from their account and notes that.
Is there an employee accident report sample?
Yes. The sample completed report above shows the template filled in for a fictional accident, with the facts in time order, the care given and the corrective action. Every name, place and time in it is invented.
