Photograph the hazard immediately

Record the name and contact information of the person who fell and anyone who saw it, while they're still on-site.

What should I photograph after a slip and fall?

Do I need the injured person's signature, or just a witness?
Get whichever signatures you reasonably can. A witness or supervisor signature alone still strengthens the record if the injured person leaves before the report is finished.
